Before Your Visit
To save time, we recommend you print, fill out, and bring the appropriate forms to your appointment, along with your photo ID and insurance card (if you have one):
Finances
We will submit insurance claims on your behalf, but payment is due at the time of service. We accept a variety of payment options and offer patient financing through the Lending Club and CareCredit. We also offer a discount membership plan.
